Sharks never get cavities because they live in water and their dense, flat, crushing teeth have a thick outside layer of 100% fluoride (fluorinated calcium phosphate) which is very acid resistant. Unfortunately, even thoughhuman teeth are as hard as shark teeth, humans get cavities because the outer layer of our teeth is made of a softer hydroxyapatite coating which is much more water soluble than fluoroapatite and is less stable in water making it more susceptible to bacterial plaque attack.
Your dentist wants both your outer enamel and the infected damaged dentin inside of your cavity, very near the tooth pulp, to repair and regrow into a super-resistant form similar to shark teeth. Until recently, there were only two effective but controversial ways to stimulate the biological process both around and under tooth colored resin composite fillings. He/she could coat the outside surface of enamel with a fluoride varnish and the inside bottom of your cavity with a medicated glass ionomer liner (GIC) which releases fluoride into your injured tooth dentin.
Controversy centers around the toxic property of fluoride. Some of the fluoride released from your inner tooth-filling liner penetrates beyond the dentin into the blood vessels of your tooth pulp, potentially accessing every organ in your body. Should this worry you? No, because the release is in such immeasurable amounts as to not likely cause any organ dysfunction. The main purpose of a filling liner is to have just enough fluoride to stimulate the pulp to remineralize and repair the infected dentin above it. The fluoride release is controlled to affect a very specific and limited area of your tooth, unlike fluoridated community drinking water which bathes the whole body, inside-out, with an indiscriminate showering of fluoride .
The main problem dentistry has had to cope with in using a GIC is the acidic nature of fluoride and not the mineral escaping into your pineal gland. Body tissues heal more naturally in an alkaline ph. Knowing this, scientists have recently developed a non-fluoride resin-modified calcium-silicate filled cavity liner, named TheraCal, that initiates tooth tissue repair in a friendlier alkaline environment.
To date, TheraCal is your dentist’s best choice of liners but you should mention it to him/her before your next filling appointment. It’s relatively new and he/she might not be aware of it’s availability.
5% sodium fluoride varnish with 26,600 ppm fluoride is dentistry’s best answer for disabled caries patients suffering with rampant caries. It’s a chemotherapeutic protective agent for the decayed teeth of both infants and adults. Varnish is safer than high dose fluoride gels, rinses, and toothpastes since the varnish seals the fluoride onto the enamel and less is swallowed.
Rampant caries in baby teeth suggests prolonged contact with infant formula, milk, or juice, typically when an infant goes to bed with a bottle. The elderly often take drugs that reduce salivary flow, predisposing to caries. The elderly also have a higher incidence of root caries because of receding gums.
Should the high concentration of toxic fluoride in varnish worry you? Probably not, if you’re caries disabled. At times, fluoride chemotherapy is the best and only choice to extend the life of a tooth. Science hasn’t yet found an effective proven safe replacement for varnish, but that should change as more people learn of the potential dangers of fluoride and start demanding safer substitutes.
